Partner Contact List and Training Records — Keep this form up to date throughout the year!
FY25 Monthly Partnership Invoice and Activity Report (PIAR) — Complete a PIAR each month for all activity and expenses occurring in the previous month.
FY25 Supplemental Invoice and Activity Report — Use this form only if you need to correct a PIAR after it has been processed by Wildfire.
FY25 Client Surveys — Client surveys need to be completed in January and July.
DES Case Escalation or Dropped Call — This form will trigger an email directly to DES.
SNAP Materials Order Form — Do you need more SNAP materials? This is the place for you.
